Use the Single Signature Sheets below if you are a registered Oregon voter, want to sign the petitions yourself, or if someone asks you to print out the petitions for them, a registered Oregon voter, to sign. (To have multiple people sign on one sheet, visit this page.)
Print out each signature sheet on white 20# paper (like Costco/Staples printer paper).
Sign your name and date at both places, then print your name and address on the last line. Do not write in the “sheet number” box in the lower right side of the page.
Mail the originals of both of your Single Signature Sheets as soon as possible to: Donna Kreitzberg, PO Box 3242, Tualatin, OR 97062-3242. Please mail signed petitions no later than June 30, 2024.
